Direct Care Worker

 Job description:

The Direct Care Worker provides individual and/or supportive services to consumers. This takes place at the Day Care Center, the consumer’s residence or in the community.

 Job requirements:

Applicants must have a valid Connecticut driver’s license, fully insured own transportation, at a minimum a High School Diploma or General Equivalency Diploma and experience working with Developmentally Disabled Individuals.  Fluent in Spanish preferred.

 Duties:

The Direct Care Worker,

  1. Follows Personnel Policies and Procedures
  2. Ensures the consumer’s individual plan is adhered to
  3. Transports consumer to medical and any other scheduled appointments
  4. Directly supervises consumer at all times
  5. Assists consumer in participating in voluntary community services
  6. Assists consumer with budgeting, vocational, educational and recreational services
  7. Teaches and assists consumer in the completion and submission of job applications, job coaches
  8. Assists consumer with light work, and works with consumer on independent life skills
  9. Advocates for client at all times
  10. Completes detailed client daily progress notes at conclusion of shift
  11. Generates comprehensive monthly client report
  12. Immediately reports incidents to Program Manager and/or Executive Director
  13. Participates in ongoing training, and attends bi-weekly meetings and training
  14. Ensures is up to date on all aspects of consumer and agency related information
  15. Participates in meetings with care giver and Department of Developmental Services case worker and performs related duties as directed by the supervisor.

 Terms of Employment:

  1. Regular Full Time or Per Diem, Direct Care Worker; work hours not flexible, Contingent to requirements of the specific contract or agency needs.
  1. The Hispanic Coalition of Greater Waterbury, Inc., Personnel Policies and Procedures (Employee Handbook) takes effect on 91st date of hire.
  1. Salary: dependent on experience and education.
  2. All Hispanic Coalition of Greater Waterbury, Inc., employees are employed on an “at- will” basis. Employment at the Hispanic Coalition is for an indefinite period of time and is terminable at any time, with or without cause being shown, by either the employee or the Hispanic Coalition of Greater Waterbury, Inc.
